At ENT Specialists, we participate with most major insurance carriers – HMO, PPO and Indemnity plans. However, we suggest you verify your eligibility for services with your insurance company. Please consult with our team when requesting your appointment. Our team will do our best to help you understand the insurance process, and assist you in any questions that you may have. Additionally, if your insurance carrier requires that you obtain a referral from your primary care doctor, please provide us with a valid referral number at the time of your appointment. This ensures that your insurance provider is aware of the services rendered during your visit.

It is the responsibility of the patient to confirm if the provider is covered under their health plan. It is also the responsibility of the patient to obtain any authorizations or referrals, required by their insurance company prior to the appointment with the provider. Please review your insurance policy regarding co-payments which are due at the time of service.
